Managed long-term care plans offer a wide array of services, including both medical and non-medical care. These services are paid for through Medicaid. In order to qualify for an MLTC, individuals must be eligible for nursing home care.

Managed long-term care (MLTC) is a Medicaid plan that helps those who are chronically ill or disabled, need Medicaid-eligible medical and/or non-medical home and community-based care to remain safely at home.

Working in conjunction with a member’s primary care provider (PCP), each managed long-term care program coordinates everything members need to stay safe and healthy at home by using a large, wide array of specialty providers in their network.
